Individuals & Families

  • No. You can self-refer or refer a loved one. Referrals are also welcome from GPs, hospitals, and community organisations.

  • We provide:

    Supportive nurse visits for people adjusting to illness or caring for someone

    • Health coaching and gentle education

    • Support navigating aged care and community services

    • End-of-life care coordination and advance care planning

    • Short-term or occasional nursing for practical tasks (e.g. medication, wound care, telehealth setup)

  • That’s a separate service and can be arranged as needed through brokered nursing or direct care.

  • Yes. We offer temporary or gap services while you're waiting for longer-term supports to begin.

  • Yes, if you’re self-managed or your provider agrees, we can provide services under your package. We’ll work with your provider to coordinate this.

  • You’ll receive an invoice after services. Payment can be made privately or via an approved package.

General Practitioners

  • We offer practical, compassionate community-based nursing care to support patients and their families. Services include short-term and transition support, home assessments, and follow-up for vulnerable patients.

    • We invoice the general practice directly.

    • You can bill Medicare for eligible services

    • Or bill your patient privately

    • Outreach nursing is a separate clinical service to assist your team

  • Yes. Outreach visits can be arranged by negotiation to Charters Towers, Hughenden, Mount Isa, Cloncurry, Ingham, Ayr and surrounding areas.

    • Help with hard-to-reach or housebound patients

    • Time-saving support to ensure referrals are completed and followed through

  • Yes, we can support drop-off to the GP clinic or assist the patient to prepare for reviews or telehealth. Or support arrangements via existing services.

Brokered Nursing

    • Self-managed Home Care Package holders

    • Organisations needing transitional or temporary nursing

    • Aged care or community teams needing skilled visits

    • GPs referring clients for short-term community-based clinical input

  • We act as an additional resource to your care team — especially during transitions or gaps. We cap ongoing clients and can join MDT meetings monthly by agreement.

  • We invoice the referring organisation (e.g. GP, RACF, package provider). In some cases, the individual client may also pay privately.

    • One-off or short-term nursing blocks

    • ACP conversations, assessments, referrals

    • Medication support, wound care, chronic disease follow-up

    • Setup for new package clients or patients returning home

    • Weekly or fortnightly visits with MDT coordination (limited caseload)

  • Yes. We offer bespoke plans — from an initial setup package to ongoing care blocks — tailored to the needs of the client and the referring provider.

  • Yes. We provide a short contract outlining responsibilities and payment structure for any service arrangements.

Pots for Pops

  • A heartfelt service where someone can gift a potted plant and visits to someone they care about. It’s not just a pot – it’s a presence. Each visit offers gentle connection, an informal wellbeing check, and support if needed.

    • Friends or neighbours

    • Local businesses or service clubs

    • GPs, social workers, cancer care teams

    • Funeral directors, hairdressers

    • Or yourself — for someone you care about

    • Anyone!

  • Both. Most pots are donated or secondhand and are lovingly refreshed. We often bring the pot plain on the first visit, and shape its story together — including painting it and adding plants or keepsakes.

  • Up to four visits. These can be spaced across a few weeks or timed for meaningful moments like a birthday or anniversary.

  • We focus on connection. Visits might include:

    • A chat and gentle wellbeing check

    • Talking through care concerns

    • Linking to community or health services

    • Companionable support after a loss or diagnosis

    • Or simply sharing a quiet moment of kindness

  • Yes. Let us know the colour or theme you’d like, and we’ll prepare it with care and deliver it on a single visit.

  • Yes. We can support with watering, care tips, or even small garden tasks on follow-up visits (short watering visits!) additional fees and on a time limited basis -e.g if the client is in hospital or carer respite.

  • Absolutely. You can:

    • Sponsor a pot + visit for someone in need

    • Donate a secondhand pot or plant

    • Contribute to our Quiet Kindness Fund to reach isolated or vulnerable individuals
