Elderly man and young woman hold potted plants

Alex was discharged after a short hospital stay. His wife passed years ago, & his only daughter lives interstate. He doesn’t qualify for in-home services yet. A neighbour purchased a Pot for Pops gift. Our visit helped monitor his recovery, link him to My Aged Care, & coordinate social support, all in a gentle, relational way.

Elderly woman looks at a gifted potted plant

Maria finished chemotherapy & was told there was no more treatment. Her family were grieving, unsure how to support her. The oncology team referred her for a Pot Visit. Over four visits, we built rapport, offered kind guidance, & gently helped the family connect with palliative care & local support.

Elderly couple hold hands while taking a walk

Brian hasn’t attended the GP clinic in over 18 months. Judy, his wife, is his carer but struggles with transport. The GP couldn’t conduct a 75+ Health Check. We visited their home, completed the nursing component, video-called the GP, & prepped the care plan for sign-off.

Pots for Pops

When delivering a pot, I asked her how her new pot would be painted if it represented her life. Joy smiled as she spoke about memories of her earlier life.

“A wild girl running freely through the hills & nature, & grasses of home“.
