About Connections for Community
Connections for Community was established by Townsville local, registered nurse Amber Ali. The idea for her reliable heart-centred service grew from seeing how many people were slipping through gaps in care. She often met patients, discharged home, newly diagnosed, or caring for someone they loved, yet to be connected to the right supports or on waiting lists that didn’t match the urgency of their situation. Families and providers would be trying their best, but the system was overloaded and confusing.
With over 2 decades of experience across community health, palliative care, primary care, mental health, cancer care, hospice, and rural practice, our team provide in-home nursing support in Townsville, surrounding areas & through remote outreach. We also provide brokered nursing, and a simple referral process for General Practitioners. Alongside her clinical work, Amber and her team run initiatives like Pots for Pops and Breaking Bread, aiming to reduce isolation and build a warm, human-centred network for those who may otherwise feel alone.
